The application form is NOT VALID until it is paid for at the CBU Cashier and a receipt issued. The receipt and the filled-in form should then be submitted at the Admisions Academic Offices or deposited in the account below;

• Include photostat of the deposit slip or anyother document showin proof of payment.
• Processing of the forms will only be upon proof of having paid.

If applicant is unable to present oneself in person, he or she should send filled-in forms with K120, 000 postal or money order or attach a deposit receipt to:
